Being watched is a paper note in Fallout 76, introduced in the Skyline Valley update.

The note can be found at Naked Creek, on an overturned Blue Ridge Caravan cart to the right from the path into the camp center.

Day 3
We're being watched. Something's been following us since we crossed into the region. On the brightside
though, we found a nice camp site. It's secluded and we found these weird chicken bird things. Taste pretty good, we've caught some more to bring back up to Big Bend after this little scoutin' venture is done.
