The posted instructions for getting past the ad.fly survey only worked for me every once in a while... well, more like twice out of like 30 tries or so.
What did work for me was the following.
1. Click link
2. Wait for redirect
3. Click back
4. Wait for countdown to start in top right corner
5. Immediately click stop before the rest of the page has loaded
6. Wait for the 5 second countdown
7. Click skip add
I doubt steps 2 and 3 are even necessary. The key seems to be to let the timer load, but stop the page loading before the redirect kicks in. Whenever I would stop it and then reload the page as suggested it would simply redirect again nearly every time.

Yes, it can show anomalies like missing chunks or flashing regions.
Yes, it can crash at any time or freeze your PC.
Yes, it can corrupt your world and delete parts of it.
But, the framerate should be stable without big spikes and the gameplay should be fluid.
No more horrible lag on sunrise or when moving around.
Even with lower FPS and a lot of chunks loading the game should be playable.
And on multi-core CPU-s the chunk loading should practically not affect the FPS.
Use it on a new world, on your own risk.
When testing keep in mind that while standing still it will try to load 3x more chunks per frame.
Edit: Does not work on multi-core CPUs. Version 3 coming shortly.
Tested the experimental on laptop with i5(2 physical cores but mc can only use 25% cause hyperthreading) and ati 5730m 1gig vram an 4gig regular ram.
Minecraft finally used more then 25% of my cpu!@!@!!
However, I have huge amount of problems with it, for one it will only show around 4 or so chunks right around me, and no matter how much I go farther it wont seem to show anymore, and basically anything beyond the very center is simply invisible, I can see all the mobs through the ground and etc.
EDIT: Also I changed pretty much every setting I could find in options and nothing seemed to make much of an impact.
Same results as toastt here. No crashing or instability, but very few chunks visible. Java 7 b147, Catalyst 11.4, Ati 5770, Intel C2D Conroe, Win7 x64.

I do not recommend patching OptiFog with MCPatcher.
And the BetterGrass fix is on the way.
If I only use the packing-feature of MCPatcher (no HD textures, HD fonts, etc.), then is that still an issue? Doesn't it then just extract all the mods into the minecraft.jar like I would do manually?
This should be OK as long as the OptiFog classes are not overwritten.

With the Firefox Addon "AdBlock Plus" I never had any problems with Ad.Fly.

Looks like I will have to find a multi-core PC for testing.
This should probably work better on multicore CPU-s.
Use on a fresh minecraft.jar and a new world.
Highly experimental, may crash or do other nasty things.

Tested the experimental V2 same results as last time it crashes when I try to change the brightness or smooth lighting and it crashes when it gets to the "Saving Chunks" bit of loading up a world

You have to load Modloader before Optifog/-mine
