The gas mask is a piece of headwear that was to appear in Van Buren, the canceled Fallout 3 by Black Isle Studios. It was also cut from Fallout 2.

Tycho's description says that he wears a gas mask. However, it is an intangible item and cannot be acquired.

Gas masks were meant to appear in the Environmental Protection Agency, but they were cut alongside the location.
Van Buren[]
Each of the NCR soldiers aboard B.O.M.B.-001 would have been wearing a gas mask to compliment their combat armor and night-vision goggles.
