Pre-Care: Welcome to FreedomCare!
Made up of our Intake and Engagement Teams, “Pre-Care” is responsible for getting things started for interested enrollees. Our HomeCare Sales Specialists are the first friendly faces (or voices) patients come into contact with, and they promote the benefits and uniqueness of FreedomCare’s services. Our Intake team guides our patients and caregivers through the start of enrollment into our program.
Onboarding: We’ve got you covered!
The function of our Onboarding Team is exactly as it sounds: handling all necessary documentation to successfully enroll/onboard our patients in a program that will change their lives. Our Onboarding Coordinators are thorough and highly dependable, ensuring that all our patients and caregivers have a solid understanding of the program and all it has to offer.

Our incredible Care Team guides patients and caregivers through their home care journey. Care Coordinators go above and beyond with a “can do” attitude to ensure that our caregivers and patients feel heard and cared for. Each and every one of our Care Coordinators approach our patients and caregivers with an abundance of empathy and attentiveness; they are the go-to people for any inquiries or concerns.
