Media Library Blank After Update From 3.9

Description

I used the Beta Tester plugin to update from 3.9 to 4.0.1 and then 4.1-beta2-30755.

While surfing the admin pages, I found upload.php almost completely blank.

Next visited the post editor and was able to see images in the Insert Media modal window.

Clicked back to upload.php, still BLANK! :O Something ain't right here!

Appears to be some kind of cache fault. Workarounds include switching browsers, and in Chrome the Ctrl+Reload combo fixed the problem.

Working 4.0.1 installation backed up via cPanel backup of SQL database and html files on Nov. 19th, 2014. Restore yields this bug.

Plugins moved to a different directory (in other words no plugins installed): no change. 4.0.1 reinstalled via dashboard: no change. Dashboard viewed in Chrome, IE, Firefox (histories cleared, everything cleared to beginning of time): no change. Core files/folders renamed, and new core files installed via FTP: no change.

The installation still renders the images when creating a new post or page, where I can still access the library via the modal interface. When visiting the site, the images are working.

It seems that no matter what I do, I can't get the media library to display the images.

I am looking for a resolution by x-mas to meet a Jan. 1, 2015 deadline.
