The primary purpose of the Nurse Case Manager is to coordinate delivery of services to managed care and Medicare residents in collaboration with the facility’s team members. The Nurse Case Manager monitors and documents the cost effectiveness of treatment provided, facilitates and coordinates the admission and discharge process, serves as the resident and family advocate and acts as a liaison to insurance and medical management professionals.

Responsibilities & Attributes
Participate in the admission process including pre-admission assessment, rate negotiations, benefit verification care needs and reporting.
Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, etc.
Coordinate delivery of services to Managed Care and Medicare residents.
Discharge follow up calls.
Assist with annual and quarterly nursing assessments.
Consult with the resident, his/her family, and the resident’s physician in planning the resident’s care, treatment, rehabilitation, etc., as necessary.
Document all payer interactions regarding resident progress.
Complete timely and thorough communication with managed care/insurance case managers to coordinate certification and stay programs.
Meet with community care teams to ensure residents care plan is maintained.
Participate in the orientation of new residents/family members to the facility.
Participate in Managed Care and Medicare Resident Meetings.
Participate in resident’s discharge planning as needed.
Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes.
Keep the Director informed of the status of residents and other related matters through written/oral reports.
Participate in developing, maintaining, and updating written policies and procedures that govern the day-to-day functions of the nursing service department.
General Requirements
Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program.
Must have, as a minimum, three (3) years’ experience in a nursing care facility, or other related health care facility.
Must have training in rehabilitative and restorative nursing practices.
Maintain licensure to meet federal, state, and accreditation standards.
WAGE RANGE: $37.97-$43.48 D.O.E.***
