Clinical Wellness Coordinator

Job Overview

  • Work Schedule: 35 hours per week –
    • Mon, Wed, Thurs & Friday from 10am - 6pm
    • Tues from 11am -7pm
  • Location: Fully onsite in the Lower East Side
  • Pay: $60,000 - 75,000, depending on education and licensure
  • Clinical Supervision (if applicable): This role offers an opportunity to receive clinical supervision
Job Summary

As our Clinical Wellness Coordinator, you will play a key role in integrating and promoting a holistic approach to services for participants of our employment services program. In this new role, you will be involved in creating new programming and engagement services to compliment our established employment, education and financial counseling services. Reporting to the program director and working closely with the employment coordinators, education specialists, and mental health clinician, you will join a team focused on close collaboration and intentional practices for holistic and trauma informed care.

Responsibilities

  • Provide one on one case management and service planning to support participants through the steps of job readiness while mitigating barriers
  • Facilitate community initiatives and events to support the communities mental health/wellbeing education and supportive services
  • Educate on and engage with the community to offer new wellness services, helping to dispel any myths or stigma associated with mental health and case management services.
  • Support clients individually or in groups to assess their needs and capabilities in order to identify strategies to meet member needs, including psychoeducation, therapy and/or clinical referrals.
  • Provide care coordination to members receiving services from other Jobs Plus mental health staff or other service providers.
  • Perform all duties within HIPAA regulations; ensure services maintain a high level of client confidentiality.
  • Complete basic reporting requirements including, but not limited to, progress notes and client tracking.
  • Manage a team of mental health clinician(s).
  • Data Management on salesforce and HSS internal databases.
  • Attend all meetings, events, workshops and training set by funder and supervisor.
  • Additional duties and projects as assigned by the supervisor.

Qualifications

  • Master’s degree in social work (MSW) required; Licensed Masters in Social Work (LMSW) preferred.
  • Must have at least 2 years managing people (interns included)
  • Must have at least 5 years working in social services, mental health, substance abuse and/or related areas.
  • Working knowledge of implementing a service modality such as C2C, trauma informed service delivery in an organization.
  • Ability to address challenges using various service modalities which include Thrive NYC’s Connections 2 Care model, case management, and make direct service provisions or appropriate referrals based on client demographic and barriers.
  • Ability to interface with funders and stakeholders to compile and analyze qualitative and/or quantitative data to inform implementation and best practice.
  • Prior experience training staff on screening tools, psycho-education, Mental Health First Aid (MHFA), and motivational interviewing and referral protocols.
  • Excellent documentation, and communication skills.
  • Client focused, solution-oriented, collaborative and takes initiative.
  • Ability to form and maintain long lasting meaningful bonds with clients.

Essential Physical Job Functions

  • Ability to stand for long periods of time during workshops
  • Able to travel to various programs throughout the five boroughs.
  • Fully vaccinated against COVID-19
