Federal rules now require all marketplace insurers to adopt a strategy for reducing health disparities and to stratify certain clinical data by race and ethnicity. Now that our HIT industry has spent nearly 40 years figuring out how to effectively pass data between sites, settings, and institutions of care (and while that journey isn't completely over, we're well on our way), we can finally turn and look farther down the road and consider what's next. In my view, advancing process consistency, agility, and change is that horizon. The BPM community is focused on that challenge, and having recently transitioned into HL7 as HL7 BPM, is squarely trying to take this on. With today's press release, HL7 is truly open-for-business in this space. Thanks to the OMG for nurturing this work along and for HL7 for having made our next home for this effort. New solutions as a result of new thinking are needed as the healthcare ecosystem grows exponentially and costs continue to rise. Where a patient has a surgical procedure done will have a direct effect on the out-of-pocket costs they experience because the cost of performing the procedure can vary significantly. SPDx is a logistics solution designed to solve a significant surgical bottleneck in the markets we enter. Surgical support trays are retrieved, sterilized, and stored ready to support the next surgery whether that occurs in a hospital or ASC anywhere in the market. Healthcare facilities can perform more surgeries than the current consignment model allows because we are more efficient operating as a sterilization factory environment. We always ensure 100% patient safety meeting or exceeding all regulatory IFUs. Staffing issues and equipment failures no longer put surgery schedules in jeopardy saving both the surgeon and patient time and frustration. Implant manufacturers get more procedures done with the same number of support trays due to increased asset utilization. This lowers healthcare costs by removing the need to deploy additional expensive surgical support tray tool sets as the number of points of care grows. SPDx is good for patients, implant manufactures, and healthcare providers. We are not a value shift; we actually create value for all stakeholders.

HCA Healthcare (HCA) 1Q24 Earnings Call Takeaways (from EarningsDigest.ai) HCA Healthcare reported 1Q24 revenue growth of 11.2% Y/Y, driven by broad-based volume expansion and improved payer mix. Facility volumes demonstrated favorable performance, particularly with increases in inpatient admissions, inpatient surgeries, equivalent admissions, and emergency room visits. However, outpatient surgery revenue declined due to calendar effects and redetermination processes, leading to a decrease in Medicaid volume. In terms of margins, HCA achieved adjusted EBITDAs of 19.3% in 1Q. Labor costs showed improvement, with labor expenses as a percent of revenue decreasing by 100 bp Y/Y. Moreover, the company saw positive trends in contract labor, which decreased by 20% from the prior year and represented 5.1% of total labor cost. Additionally, supply costs as a percent of revenues improved, while other operating costs remained relatively consistent for the past three quarters. The company reported robust segment performance across its hospital portfolio, with 56 of its hospitals showing growth of greater than 10%, and another 1/4 of the portfolio growing greater than 5%. Inpatient admissions, inpatient surgeries, and equivalent admissions demonstrated strong growth metrics across all divisions. Furthermore, improvements in service line volume were observed, including growth in obstetrics, indicating a broad-based performance across various segments. Commercial volumes, representing approximately 36% of equivalent admissions, contributed positively to the overall segment performance. Management reaffirmed its full-year 2024 guidance ranges. The call also highlighted several risks, including: 1/ Medicaid Redeterminations: HCA noted a significant decline in Medicaid volume attributed to the redetermination process, which could potentially impact patient volumes and the payer mix going forward. 2/ Operational Cost Pressures: The company is experiencing upward trends in operating expenses, particularly in professional fees and provider taxes related to supplemental payment programs (even though margin has remained relatively consistent for the past three quarters). 3/ Outpatient Surgery Volume Decrease: Total cases for outpatient surgery declined by 2%, primarily due to unfavorable calendar effects and the Medicaid redetermination process. With the elimination of non-compete agreements (temporary?), there has been rampant forecasting and debate about its implications in healthcare. Frankly, healthcare often is a negative catalyst industry, meaning it needs a negatively disruptive event to motivate innovation adoption (look no further than Covid or the IRA). As a healthcare business builder and investor, I am eager to collaborate with with founders in both vertical solutions as well as horizontal technologies. By horizontal, I mean solutions that every payor/provider/manufacturer must perform frequently/daily. A lot of the new AI companies focused on healthcare have taken aim here. I find vertical solutions just as compelling. By this, I mean healthcare solutions that these healthcare nodes don't NEED to own because the node does not need full utilization of the asset or even the base load demand required to economically justify the investment. But historically, for reasons not entirely quantitatively driven, these nodes choose to invest in the product or service. I forecast that the elimination of non-competes will stop or at least substantially curtail this negative ROI practice. Case in point, my good friend, Dr. Sina Haeri, MD, MHSA is building Ouma Health (note, I'm not an investor here). Sina is one of the leading clinicians in Maternal-Fetal Medicine. This is a relatively small but life saving practice area. Ask any mother that has faced a high risk birth. NIH indicates there are only 1355 MFM docs in the US, with a significant concentration in six states. Given the scarce skillset, maintaining MFM capability is extraordinarily expensive for any one hospital system. At Ouma, leveraging the combination of expertise and tech, Sina and his team are making the service available to all hospital systems/payors/self-insured employers on a semi-variable priced basis. Brilliant - a win/win for everyone, and clinician uptime is maximized! And now, Ouma's customers (aka providers) aren't forced to figure out how to maintain/pay the fixed cost for sub-optimal utilization of their MFM practitioners. Economically, this is the necessary business model. And as we look out over the next 2-5 years, given their expertise and accumulated data, models like Ouma will further distance themselves because they can build and augment with AI assistants for further capacity in a non-linear expense framework. Exciting times!

$1.6 billion. That's what UnitedHealth Group projects as the total earnings impact in 2024 of the February Change Healthcare #ransomware attack. This includes both direct response costs (e.g. platform restoration and increased medical care expenditures) and business disruption impacts (e.g. lost revenue). The figure is now so large that it shows up in the net earnings calculation as part of the first-quarter 2024 financial results announced on Tuesday. And remember that UnitedHealth hasn't yet disclosed the amount of personal #data breached in the hack - it needs to do this within 60 days of learning about the breach, and that time expires next week. The damage could get even worse. More needs to be done to detect and prevent ransomware. 10-figure costs should not become the norm. If you're a #founder working on a new idea, please reach out!
