We're underestimating how physicians will build and access software in the future and we're 100% underestimating how this will impact independent practice. By now I'm sure you've seen the Claude 3.5 Sonnet demos and all of the use cases or application non technical folks are building with 1-3 line prompts. Even Jensen Huang, CEO of Nvidia made a controversial comment on the future of coding, “It is our job to create computing technology such that nobody has to program. And that the programming language is human, everybody in the world is now a programmer. This is the miracle of artificial intelligence,” This is absolutely true, and if there's one area where this will have the biggest impact on society, it will be healthcare. The reason for that is that physicians and clinicians have NOT been included in the design of healthcare technology over the last 20ish years, leaving them to practice in systems designed by non-clinical people. Claude 3.5 offers a glimpse into the future of independent practice. Physicians will be able to design, build, and launch AI applications as needed without requiring developer support. In the future, I envision independent practices hiring their own AI Agent developers who can not only build these applications, but also integrate them into their practice operations infrastructure to create the ultimate practice management system. The benefits of DROPPING BlueCross BlueShield as a payer.... One of our clinics at StrataPT was seeing 3,000 BCBS visits. Here's the result if they dropped them right now - Save $300,000 - Drop 1.2 full-time employees - Downsize clinic size - Pay less rent - Profit margin goes from 9-12% to 20-25% Now if they successfully negotiate their BCBS contract up even $10 it's a totally different story. One of the most underrated aspects of using an #interoperability tool like Metriport is not needing to know where the patient has received care previously 👇 While this may be obvious to providers who have leveraged Health Information Exchanges (HIEs) before, the reality is that so many providers today still need to ask their patient where they’ve received treatment previously (assuming the patient remembers them all), and then needs to call each facility and beg them to send over the records via #fax. This process can take days, if not weeks, and often still results in the provider having an incomplete, partial understanding of their patient's medical history 🤷‍♂️ Through #HIEs, a provider who leverages Metriport simply needs to enter the basic demographic info of their patient (name, DOB, gender, and address) to pull records from all the facilities where their patient has received care previously. Metriport handles this entire process in seconds, without any humans in the loop. This is by far one of the biggest “aha” moments for providers who use Metriport for the first time, after being reliant on manual retrieval methods previously. From initial skepticism, to providers who now can’t live without this, we see these tools as an inevitable part of evolution of modern #healthcare workflows.

There are plenty of ways we rely on interoperability in our daily lives – and we never think twice about it. Like sending a text message that is received by your daughter – that shows up exactly the way you sent it. We only think about interoperability when it doesn’t work and creates frustration for users – like the green bubble versus blue bubble text message debate. It’s no big deal when the photo of your dog is pixelated when it makes it to your friend. But when it comes to the exchange of your health information between your doctor, pharmacist and insurance provider – the consequence of incomplete information risks the safety and quality of your care. We’ve come a long way in healthcare interoperability – but we’re not there yet. REULTS UPDATE… Last week I posted about using AI to prep for a conference in a new industry segment. Here’s how it worked out: - 15 conversations w/ target personas (Hospital executives, C- and V- level) - 2 were pre-arranged (cold outreach on LinkedIn) - The other 13 came from finding people on the spot at the conference (more on that below) - 14 new LinkedIn connections (all whom I’ve met in person) - 4 requests for follow up meetings with our company Here’s what I learned from this process: 1/ Have a purpose - I knew who I was looking for and what I wanted to ask them - Trying to do this on the fly doesn’t work well - Have a plan, so when you’re in the moment you can just execute 2/ Better questions = better conversations - This is where the AI driven prep was key - Asking about specific topics and trends shows credibility  - Follow up questions are also key – going a layer deeper to keep it going 3/ Minimize “safe” conversations - Going to booths asking “so what does your company do?” isn’t optimal - Finding a CFO and walking up to them is more uncomfortable (at first) but more beneficial - Everything has its place – but spend the most time on the most valuable things 4/ Have a short memory - A few people blew me off – oh well - I like to joke about this to stay in the right mindset – “That guy clearly had more important things to do than talk to me” - The psychology of live, cold intros fascinates me - It’s one of the most beneficial life skills I’ve learned, even though it took until my late 30’s (I could write an entire post on this alone) TL;DR - Go in with a plan - Do your homework - Spend time on valuable stuff - Don’t take getting blown off personal I hope this helps someone. Especially those earlier in their careers and/or new to conferences.

There’s a category we believe is getting too little attention when it comes to AI in healthcare: AI Clinical Documentation Integrity (CDI) While AI ambient scribes and autonomous medical coding solutions have gotten most of the headlines, we’d argue AI CDI is equally impactful when it comes to quality care and accurate billing. For those not familiar, CDI ensures that clinical documentation accurately reflects the patient’s medical condition and the provided care. Historically, CDI was a manual slog, with dedicated CDI specialists, nurses, and physicians painstakingly reviewing patient charts to ensure documentation supported the care delivered. Today, AI is reshaping this landscape. While it’s still early days, we’re seeing three primary approaches to applying AI when it comes to CDI: 1️⃣ CDI + ambient scribing—an AI ambient scribe and AI CDI assistant operate simultaneously, generating comprehensive notes and surfacing codes for physicians to review at the point of care. (Example: Ambience Healthcare’s AutoCDI) 2️⃣ CDI via real-time diagnosis support—diagnoses are suggested at the point of care based on clinical data in the EHR, with auto-generated documentation and audit trails. (Example: Regard) 3️⃣ CDI via an AI-driven post-coding / pre-billing review—the entire patient record is ingested and analyzed against what is coded, surfacing suggestions for revenue and quality opportunities. (Example: SmarterDx) To date, AI in healthcare has focused primarily on administrative use cases. With the increasing adoption of AI CDI solutions, we’re now taking a step closer to clinical workflows. As clinicians get used to AI-suggested diagnoses in the CDI context, it will be interesting to see if this increases their comfort with AI playing a role in clinical care. The US hospital information system market is a $60 billion industry...but most tech vendors I talk to have a fuzzy idea *at best* of the true potential of their content marketing. They often under, or OVERestimate what's possible for their sales and marketing department. The #1 mistake we made with outpatient technology over the last 20 years was thinking that every EHR had to be built for a specific doctor, clinician, or practice model. This was wrong and it only led to more fragmentation, less interoperability, and forced everyone to rebuild the same software over and over again. As the shift back to private practice becomes more apparent to founders, I see a similar pattern emerging as they rush to solve this problem for the millions of doctors and clinicians who will no doubt be looking for a solution. However, if they're building private practice for X they're simply repeating the same mistakes of the past which will ultimately lead to failure. Care delivery software is one of the best examples of where the 80/20 rule actually makes sense. No matter what the speciality or patient focus is, 80% of the core functionality is ALWAYS going to be the same. Therefore, we don't need different platforms for different specialties, but instead we need modularity that allows faster application adoption in order to meet the 20% of customizations doctors and clinicians require. This means the ability to quickly build and deploy applications is the real missing link. Through AI, we will be able to achieve this 20%, as it will allow practice owners to quickly develop or integrate direct solutions that are specific to their clinical workflow, using coding AI agents. Outpatient private practice in the post-AI era should place a greater emphasis on building and training AI to manage tasking and reasoning decisions based on patients, specialties, or business models rather than creating multiple care delivery platforms for X as we did with EHRs. Here is a summary of CCHP's latest update on Out-of-State Telehealth Provider Policies: Provider licensure exceptions for telehealth across state lines remain a key issue, with frequent inquiries from providers and patients. Providers often seek a list of states allowing out-of-state care via telehealth. Generally, telehealth is governed by the patient's state, requiring providers to hold an in-state license or approval, with some limited exceptions. Limited licensure exceptions vary widely by state and profession, making a comprehensive list difficult. Common exceptions include provider-to-provider consultations, continuity of care, infrequent interactions, emergencies, and specialized care. Currently, 29 states have such exceptions, with 9 states also having a telehealth registration process. Telehealth registration processes exist in 21 states, aiming to ensure oversight of out-of-state providers, often with restrictions on in-person care. Interstate licensure compacts also facilitate out-of-state telehealth, with only 5 jurisdictions not participating in any compact. There are twelve compacts, each varying by provider type, aiming to streamline multi-state practice. Providers should always check with state licensure boards for compliance, as state policies and insurer coverage requirements vary. As of June 25, 2024, CCHP's data shows: - 29 states with limited licensure exceptions - 21 jurisdictions with telehealth registration processes - 9 states with both exceptions and registration - 12 jurisdictions with no specific exceptions or registration, 9 of which are compact members - 5 jurisdictions with no compacts - 3 jurisdictions with no exceptions, registration, or compact https://lnkd.in/gspndAWG
