Slowing down page and not loading.

Not sure what is going on here. I have used the form on a few other domains and no issues. But for some reason it is not wanting to work on the above page. I have tried going in and saving all of the stuff and reloading on the page. I have tried turning off the app and reactivating it. I have tried downloading a new version and uploading it. I have copied the form and pasted the short code on a blank page with nothing else. Nothing seems to work. There may be a conflict with another plugin but I have not recently added any new plugins and do not know how to check that. So any advice, suggestions or insight would be great! Thank you.
